When something goes wrong, how we respond defines who we are. This procedure ensures every incident is handled with urgency, transparency, and care for the people involved.
Procedure Steps
Follow each step in order. If something does not apply to the current situation, note it and move to the next step. If you find a better way, bring it to your group leader so we can improve the procedure for everyone.
Secure the scene immediately. Stop all work in the affected area. Ensure no one else is at risk.
Attend to the injured person first. Call emergency services if there is any doubt about the severity. Do not hesitate.
Notify the Field Commander and the Watchman within 15 minutes of the incident. Do not wait for details — report what you know.
Document the scene before anything is moved: photos, witness statements, conditions. Details matter.
Complete the incident report form within 24 hours. Be thorough and honest. This is not about blame — it is about learning.
Conduct a root cause analysis within 48 hours. Involve the crew who were present. Ask 'why' five times to get to the real cause.
Implement corrective actions immediately. Do not wait for a committee. If you can fix it now, fix it now.
Follow up with the injured person. Check on them. Show them they matter to us beyond the jobsite.
Share lessons learned with all crews at the next safety briefing. Every incident is a teaching moment for the entire company.
